Application/Confirmation page
When booking your visa appointment, you must ensure that you, or your representative, submit a properly completed DS-160 form (online visa application).
A properly completed DS-160 form contains the complete and correct information (including name, sex, visa class, date of birth, passport number, et cetera.) for the applicant who will appear for the interview.
Please note:
In order to attend your appointment at the Visa Application Center (VAC), you must bring your original DS 160 confirmation page that was used to schedule your appointment.
If you have submitted a new DS 160 visa application after scheduling your appointment, you must bring the original DS-160 confirmation page you have used to schedule your appointment AND the corrected new DS-160 confirmation page to the VAC or document drop-off location on the day of your appointment. If you are unable to bring your original DS-160 confirmation page and your corrected new DS-160 confirmation page, you must cancel and reschedule your appointment by entering your corrected new DS-160 application number.
Please note: If your visa fee receipt is already expired or has been used to its maximum attempts, you must cancel your existing appointment, pay a new visa fee, and schedule a new appointment using your latest DS-160 application number.
